169.06 of the Ohio Revised Code, you must possess a property interest in an unclaimed
account in order to request a claim form. A property interest can include ownership
or a right to the unclaimed funds; however, you may also request a claim form for
a family member or friend. If you request a claim for a family member or friend,
please enter their name and current mailing
address on the next screen. If you choose to print this form on your own printer,
be sure to forward the claim form to them. If you are unable to print the form,
the Division will mail it directly to them. Including the correct information will
speed the processing of the claim.
